They both are.

(But they - typically - occur more with applications for *extensions* of expiring patents, not with brand new patent applications.)

In either case though (tacking more years on to a patent that would otherwise expire on schedule, or else granting a new patent application) the economic effect is the same: delayed emergence of lower-priced competition from generics and bigger profits for the original patent holder from the protected market.
