SISKINDS now has the VRS information form on their home page I just filled out the class action suit form at SISKINDS. Simply query www.siskinds. Right in the middle of their front page is the VRS information form. Please check it out. It looks like they are taking this very seriously. In the info form they ask you to fill in how many shares were purchased and when. Below is the cut & pasted article they've posted.
