You wouldn't believe how bad things are. At the current levels of reimbursement - and with the current cost structure, including salaries to support personnel and billers, rents, insurance etc. - It makes no difference if 110% of the population will have insurance. A doc who makes a living providing so called "cognitive services" - that is, using his brain rather than do procedures - would still not survive in a practice setting.

Not long ago I had a chat - at midnight - with a doc in his 50's, who was still in the hospital finishing the day's business. At 7.45 next morning he was expected to start his office hours. He said such long days happened quite often. He is employed by one of the local groups, works his a$$ off, and most years makes under 150K. He thinks that if he breaks away and goes solo, he'd be making much less.

....And yes, I am blaming for this our government, which was messing with fees - and increasing costs - for decades.
