The so called "concierge" arrangements are reminiscent of retainers. Patient pay annually a pre-agreed upon amount, and then fee for service if needed.

Everything takes time and work. The only truly simple arrangement I've seen was the one I mentioned here a few times, with that old doctor getting paid his moderate fee during the visit, and then patients would bring - about twice a year - filled out forms, and he would sign them.

I don't think his quality of care was in any way lower than in elaborate places with big wasteful front- and back business offices.
