The so called "concierge" arrangements are reminiscent of retainers.

I had a concierge doctor for maybe five years. No retainer. I just handed the front desk my credit card every time before leaving the office. How I paid my credit card bill was my problem. Now that's really doing things the old fashioned way. <g>

I think the way most of them work is to just charge a flat annual fee. That's pretty simple and I guess quite workable if they can get the fee right.
