I tend to trust Harvard studies. I haven't looked into exactly how the determine things... but my understanding is they take into account treatable illnesses and injuries that were neglected too long or never treated at all that led to a death that with care could have been avoided.

a simple example is someone with flu who never goes to the doctor or doesn't go until it is too far along. these things can be tracked. and I don't think most people (not all) who have healthcare understand the choices the uninsured make in terms of their health.
