I would say that a person who can't stick to reality, and instead goes off on paranoid tangents about her own invented scenarios when discussing an issue is having trouble functioning. When one reads transcripts of her radio and television appearances, one is struck by how often she seems to lose track in the middle of a sentence, coming out with some pretty garbled stuff. That's another example of having trouble functioning. - Allen
