One fact remains clear: We have failed miserably to insure 100% of the people of the United States of America. There is no need for the government to provide health care. But there is clearly a need for the government to be involved in making sure that insurance is provided that covers every single man woman and child in America. A means tested approach is typical of places that cannot come to terms with the nature of the issue. You want maximum bureaucracy? You want confusion? You want administrative complexity? Then go the means tested route -- you will get what you deserve -- high costs and poor health care coverage.
