So, the 21% cut is merely a tactical manipulation meant to help pass the bill by (temporarily) lowering the price tag... Man, our ruling political classes are becoming a mess.

I think practices will be shutting down even if there is no cut. There will be an interesting demographic phenomenon taking place over the next few years, as more baby boomer primary care docs will be heading into their 60's.

Traditionally, older docs would slow down, start seeing fewer patients, work part time - and most of them would continue working for many more years, many into their 70's and sometimes beyond. Nowadays, this is no longer an option - you need to work full blast just to keep the doors open. There is no such thing as slowing down, you won't earn enough even to cover the overhead. Therefore, many more will retire than "normally" would.

I predict there will be an unexpected shortage of primary care docs. Many will find niches, like supervising nurse practitioners or working as part time employees... but there will be some serious changes coming our way.
