You are right that commercial insurers use Medicare more and more as their guiding benchmark. The difference is that doctors are not required by law to accept their rates. It's generally smart to discuss costs and billing issues in advance, and maybe do a little comparison shopping.

It is way too expensive to practice medicine these days, or to run a hospital. In order to cut costs we need to simplify, to DEREGULATE. Instead, they think they'll solve the problem through ever tighter controls and micromanagement. It will not work well.
