Not going to buy the paper (abstract doesn't help) to verify this, but an abstracting newsletter that I get ("physician's first watch") indicates that responses occur in one of four with very severe depression.......

Antidepressants' Benefits Appear Greatest in Those with the Most Severe Depression

Antidepressant drugs have "substantial" effectiveness in patients with very severe depression, but may be ineffective in those with milder symptoms, according to a JAMA meta-analysis.

Pooling patient-level data from six randomized, placebo-controlled trials comprising over 700 patients, researchers examined Hamilton depression scores at study intake and at the conclusion of treatment. After a minimum of 6 weeks' treatment, patients with the highest Hamilton scores at entry showed the highest effect sizes from treatment.

To produce a clinical improvement, the authors calculate the following numbers-needed-to-treat:

16 in mild-to-moderate depression
11 in severe depression
4 in very severe depression

They advise clarifying to clinicians that while antidepressants "can have a substantial effect with more severe depressions, there is little evidence to suggest that they produce specific pharmacological benefit for the majority of patients with less severe acute depressions."
