My question was an attempt to find out if doctors who opt out tend to set up alternate programs such as concierge service or private contracts.

I don't think that a doctor can just send a Medicare patient the bill for a service that Medicare would pay for were the doctor enrolled. So I don't think the letter could say "pay yourself." But I'm not sure.

It's really hard to find info on such things. Sources on the WWW tend to be gated for professionals.
