I would think that would be highly unusual.

AFAIK there is nothing that would stop a doctor from seeing a Medicare pt. for cash if he were not a Medicare provider (participating or non-par). If he is a Medicare provider, he has to abide by Medicare's rules. But if he isn't a Medicare provider he can do whatever he and the patient agree to.
