Yes. More importantly, it's too simplistic to blithely suggest that Medicaid should be expanded to take in more of the uninsured poor. Somewhat similar is the idea that Medicare should be expanded to take in uninsured 50 year olds.

The various levels of reimbursement make both of these ideas rather unfeasible.

We just have a crazy patchwork system of coverage and payments in which cost shifting is what holds it all together.
