But they can and do run out of money, at which point a catastrophic policy doesn't help them. They have to be able to pay the deductible.

Which is an issue for those who have already run out of money for this reason, or for those who are close, but not so much for new people (people who develop these conditions that are then "high risk" or "preexisting") after the scheme is set up. They would be covered for the medical catastrophe, so they shouldn't run out of money, at least not for this reason.
