Hospice is care for those not expected to be cured. One problem that crops up is that few doctors will look at the big picture of all the drugs different doctors prescribe. It takes one person to coordinate and make sure that the patient isn't getting two things with similar effects. Pharmacists should identify incompatible interactions, but the opportunity to use one drug to address multiple symptoms is often missed.
