Severe chromosomal instability of telomere crisis promotes secondary genetic changes such as end-to-end chromosome fusions and non-reciprocal translocations that facilitate carcinogenesis.

In other words, the cause and effect are chromosome instability (critically shortened telomeres) and increased incidence of malignancy, respectively.

This phenomena is somewhat paradoxical and being discussed at the last paragraph.
