The biggest cost problem with the medical providers is moral hazard.

Could you expand on that.

The only way to deal effectively with moral hazard is to get away from fee-for-service and low co-pay policies.

I understand the low co-pay part, but moving away from fee-for-service, would seem to increase the desire at the margin for people to get more health care, for which they won't have to pay any additional fee anymore.
