The current bill is so silly that it only makes sense as a stepping stone towards something else. Let's go over a few points.

They're cutting Medicare... What else is new?

Medicaid will get expanded, and some of the control will get taken away from the states. Probably, along with some of the costs.

The penalties for not having insurance are rather moderate. Together with a ban on "pre-existing condition" exclusions this represents an invitation for people to hold off buying insurance until the person gets seriously ill - at which point the insurance company gets slammed.

If things do not run smoothly - and they will not - then, the "public option" provisions will come in to save the day. Expect the existing commercial insurers to become "carriers" for the public option, at least at first. The government has no clue whatsoever how to run a health insurance operation, and will look to farm out the "public option" to those who know how to do it. Private insurers will administer it, but with a growing involvement on the part of the government.

Gradually, the public option will indeed evolve into a government run utility. After that, the next logical step would be for most healthcare workers to become government employees.

Along the way, the government gets a justification for taking in a shipload of money in new taxes.

I think, that's the plan.
