If its voluntary than its charity. If its forced, its confiscation of wealth, effectively government backed theft or extortion.

Insurance companies write policies knowing that a certain percentage are going to be money-losers.

Yes a certain percentage, not any particular claim. Whether each person is going to be paid is unknown. Insurance is the spreading of this risk. If the person already has the condition that will have to be paid for it isn't a risk, its something that already happened.
