No.

That not buying something is not commerce, and that in state actions are not "interstate".

I wouldn't oppose the idea that commerce as understood by the framers of the constitution wouldn't include medical care, but I wouldn't support it either, or most other attempts to claim that buying and selling items is something other that commerce for purposes of considering and applying the constitutional limits to federal power.
