Don't know enough about the inner workings to make an informed comment.

The standard premium is $96 per month. At $85K it goes up to $155. At $111K it goes up to $221. At $160K it's $287. Above $215 it's $354. Like I said, that's not adjusted gross. It includes tax-free income, too, which they conveniently add back in for you from your 1040.

Does that match up your sense of what constitutes "rich" or not and an appropriate penalty for it? What else would you need to know to make that judgment?

(I was mistaken in my previous post when I said "double." I had in mind that the premium was $175 at $85K, not $155. It's "only" 63% more.)
