Personal responsibility would be not having Medicare in the first place. I don't think your really calling for that.

The argument that its ok for the government to outlaw alleged unhealthy activity or substance X, because it might increase costs for the government is the road to creeping soft totalitarianism (and hopefully it stays relatively soft), esp. in the context of the government's growing involvement in healthcare.

If you think the government shouldn't pay, than argue it shouldn't pay, the person with diabetes isn't making the government pay (or he or she is only one vote in the process), the government is implementing a policy that it will pay. If I take someone's money and give it to you does that mean I get to control your life? Of course not. The same should apply to government action.

after all the insurance companies charge more if you're a smoker

You can choose not to pay their premiums, at least you could before the unconstitutional Obamacare mandate.

There is a vast difference between government force, and voluntary private contracts.
