"What I don't understand is why they can't get the underwater valve to shut."

The problem is, there isn't an automatic way to shut off the valve if it doesn't get triggered initially. Other countries require a backup, and we considered such a requirement a few years ago(2003), but the oil industry persuaded them they weren't necessary. Given that was the Bush administration, well...

Bush, the gift that keeps on giving well after he's gone. I didn't know that........I can not believe there is not a manual way to shut the valve off. That is standard practice in most industries.

"Having mechanical failure at this relatively new stage is worrisome."

What is even more worrisome is that there wasn't just one failure, there were two. The BP stack can be triggered by a wire that runs down to the switch. That failed, or it could have been that the fire blocked the personnel from being able to activate it. In addition, there is a deadman switch that is supposed to trigger it if contact is lost with the platform. That failed also. Now it could be that the BP stack itself failed. But, they incorporate two different systems. Given their construction, it is hard to imagine how that could have happened.

I have stayed away from this issue because its not clear if the personnel got to the switch before they perished. Needless to say, whether they did make it and the switch didn't work, or didn't make it because of the fire, its not a very good safeguard measure. It explains why the Norwegians and Brazilians use a second switch on a boat.
