According to one of the survivors, 2 times the oil/gas tried to break up through Halliburton's concrete plugs. They call it a burp. First burp is supposed to cause the operator to shut the fail safe on the BOP switch.

Some reason BP didn't do that (cause they wanted to save the well?).

Then it burped again, then it fried 11 men on the drilling deck including the 2 operators who had access to the BOP switch when it broke the plugs and sent a gusher of oil/gas right up thru the drill deck.

Word is BP gave Hal incorrect info on the well depth(deeper than the 18,000' reported) so when Hal filled in the concrete they didn't have enough for the 160,000 PSI pressure thus the blowout.

A strange metaphor when you consider the CEO of BP is ex-GS, no?
