Not for the first time I had to wonder why Americans are eating themselves to death

You can attribute that to the free competition amongst restaurants, who try to get more customers by offering more "values" in the form of bigger portion. There restaurants mostly cater to middle to lower class segments of the population. You don't get that big portion in upscale restaurants since the quality - and not the quantity - of the food that is important.

Just observe the crowd. The percentage of rich people who are obese is smaller than the people will lower income. I don't go to buffet anymore since it turns me off to see way overweight men and women piling food on their plates. And when they finish and can't take it anymore, they go to the bathroom and don't even wash their hands afterwards. Really disgusting.

I like the SIngaporean rule: you don't wash your hands after you finish with nature calls, you pay penalty. The main reason is that Singapore is a small country and she does not want germs spread from contaminated hands touching public areas, and especially door handles and knobs.

As a nation, we can learn from Singapore. That's one aspect of staying healthy.
