Roughly half of our health care economy is split between two payers (Medicare and Medicaid). The other half is split amongst the insurance companies.

The heart of our problem is with the government side. Two basic problems with the government side --

a) Our government is big, bloated, inefficient, and incompetent at tasks like evaluation and payment of claims. As a result, we have massive problems with fraud and bureaucracy, and

b) The concept of government run health care in our environment is severely flawed, as there is a constant battle between public funded (in which payment is established by law) and private insurance (in which payment is established by a combination of factors, partially competitive). So, government programs reimburse a fraction of what most private programs reimburse.

We are now seeing the fracturing of our system into public versus private (not a new thing, but the pace of the fracture is quickening), and those on publicly funded programs will receive inferior care as a result of providers abandoning the public payment system, which doesn't adequately compensate providers.
