I always hear that the two major problems in U.S. health care are:

* High costs. For instance, ICU care is many times more expensive than equally performing ICUs in other countries, which are better staffed and have similar wage levels.

* Sometimes patients don't get treatment before the insurance has approved it.

I assume that Medicare and Medicaid are cheaper than private insurances, which indicates that you have a different perception of what the problem is, in U.S. health care. Is this correct? If yes, what do you consider to be the biggest problems in U.S. health care?
