I think, dybdahl, that you may be overrating the financial aspects. They are important, like in everything else in life, but they are not the only thing. You created a long list of hypothetical pathways, and you seem to imply that specific choice of action will depend on money. I submit that nothing is further from the truth. Unless your decision maker is totally - maybe criminally - corrupt, the main factor will be the patient's needs.
