You can get better health in many places: Good restaurants, fitness centers, swimming pools, glasses shop etc. But that is not health care according to common definitions.

A normally healthy person does not need a doctor or a nurse. There are some public health advices that might make you go to a doctor, but that is usually based on national economic motives.

Cosmetic surgery is usually also not health care, but it is also the best working part of the medical industry.

So, health care is basically about fixing illnesses. The more ill you get, the more health care costs. In many contexts it makes sense to measure illness in health care dollars.
