Part of the problem is defensive medicine. Radiologists get sued if they miss an early tumor on a mammogram; therefore they tend to overread, which leads to more (maybe excessive) biopsies. When the mammo is read as being "high probability", quite often it's the real thing, and a resection can be life saving.

In pursuit of perfection - and in fear of missing a relatively small number of cases - we overreact in too many cases. This leads to an increase in morbidity and mortality due to aggressive evaluations, and destroys the overall effectiveness of the screenings.

The same is true about prostate cancer. As far as screening colonoscopies are concerned - I think the advice for everyone over 50 to have one is very sound, whether or not it's "cost effective".
