Comon Buddy, your smarter than that.

I did say "total costs", which means all costs. I said it in the context of a discussion of health care costs in the US, so obviously its all costs in the US (I suppose it could be argued that it would include costs the US imposes on the rest of the world, but they would be minimal, so that point would be an unimportant quibble).
