I think financing Medicare for All via a sales tax is pretty progressive. The rich are spending a heck of a lot more than the poor. Having the poor pay nothing for their medical care (like the Obama bill does) is not a good long term strategy. It eventually turns not only the rich but also the middle class against the poor.

There is little opposition to the welfare state in Europe. I think the main reason is that the poor participate in its financing via a hefty VAT.
