In general, you just request it from your sending broker (the broker sending the shares). You'll need the DTC account number to the receiving broker (the broker receiving the shares). They're supposed to take not more than 3 days. It's my understanding, and I've never seen it disputed, that it forces STREET->NAME share conversion immediately.

Specifically... I faxed this to E*Trade:
