I caught a few minutes of an interview with a Arizona Resident who needs a heart transplant and Medicaid won't pay for it there.

I thought, "I wonder what the sequence of events was here? I bet he had not bought any Insurance and when he found out he was very ill he applied for Medicaid. What choices had he made in the past? Was he paying a $60/100 a month cable TV/Roadrunner bill for entertainment rather than buying Health Ins?"

Cynical me says that what was going on.
