I don't understand what you are talking about there....

The state only issues *one* form of birth certificate at any given time.

But, since the 'sixties, Hawaii like many other states, has changed it's document formats SEVERAL DIFFERENT TIMES.

(Still... at any point-in-time only ONE FORMAT is issued.)
