The main problem is that all these guidelines are really meant for the physician who is not a "real" HIV Specialist. They need some guidelines to help these patients. The truth is that unless an HIV patient is cared for by someone who's practice is at least mostly HIV, they will always be behind. I start patients on treatment asap, if they have virus circulating, that means that active replication is taking place. The longer you wait, the lower the CD4, the more damage to the immune system. And we know how hard immune reconstitution is. Once you lost a memory cell, you will most likely never get it back. Hopefully these will show doctors, manage care orgs, and others that they are way over their heads, and that Special Immunology is a Specialty. JLL
