She mentioned that the cap of the reactor blew up and 211
graphite rods melted, creating a big fire in Chernobyl that
was put out with sand. In other words, that was a complete
melt oudoors.

It pretty much happened right after the "error".
So far the scale is much lower in Fukushima, a few
orders of magnitude lower. We did have some melt there,
obviously.
