War (or battle or military operations, if your definition of war only includes larger operations and/or declared wars) is a violent, somewhat (and often very) chaotic activity where all sorts of mistakes and accidents happen. It has always been that way, and perhaps always will as long as their is war (which will probably be as long as we are around, or even longer if you include AIs or aliens fighting each other).
