I actually got a response to an E-mail message that I sent to the marketing research company regarding the survey. Regarding the source of developers:

"The majority of the developers surveyed were selected at random
from among US companies. The sample was supplemented w/ a database of
attendees at various Java conferences held over the past 12 months."

They declined to identify the particular client that requested the survey (they said that they have "a variety of clients" for whom
they collect the information).

They also said that they used both closed-end questions (e.g. "What Java tool do you use most often?") and open-ended questions (e.g. "What do you feel is the number one thing lacking in the tool you use?") . They said they can't release actual survey information without approval from their clients (the clients own the data).
