It is inarguable that commercial insurance operations are more efficient at running their operations than is either Medicare or most state Medicaids. Furthermore, commercial operations do not lose 100s of billions to fraud and abuse every year as these government operators do.

But it is important to keep in mind that it is government involvement that mucked up the works to begin with. By having government set provider fees by fiat rather than allowing markets to set pricing, you totally lose any sensible market-based economies. It becomes a test of wills to see how the system can best be manipulated to avoid dealing with patients from government-run operations. So, even Medicare patients end up having to purchase supplemental policies if they want to have good health insurance coverage.

While we do have expensive health care, it is also the best available anywhere when measured by any objective criteria.

So, I guess you would say that every point you made is just totally wrong.
