It is inarguable that commercial insurance operations are more efficient at running their operations than is either Medicare or most state Medicaids.

That is not only very arguable but it is a documented untruth ...

I've posted these facts here time and again. You could not possibly have missed it. It costs AT LEAST 30% more to run Medicare than for any commercial insurances.

If you have documentation, let's see it.

So, even Medicare patients end up having to purchase supplemental policies if they want to have good health insurance coverage.

That's the biggest pile of non sense...

Straight Medicare and Medicaid patients today are being shuffled off to PAs and APNs at an alarming rate. While MDs continue to see commercial insurance patients. That is a fact, one I see every single day in my work and one that has been well reported in the media since Obamacare passed.

I, and many like me in the same business, advise physicians to do precisely this in order to eliminate the drag from being topheavy in government programs. The alternative is to quit seeing these patients, who can't even cover the cost of their visits, altogether.
