Because the payment system is so screwed up it encourages the wrong kind of behavior. Rather than APNs and PAs being used for cases that are appropriate for their knowledge (and the supervision they receive), they are being assigned cases based on the fact that the patient is Medicare or Medicaid.

While I don't believe those with government coverage ought to necessarily have the SAME level of care as commercials, they ought to be assured of an appropriate level of care in any given circumstance. As it now stands (and this is getting worse by the day), these people are being sent to see inadequately supervised APNS (and to a lesser extent, PAs) without consideration of their medical circumstances, giving only consideration to their insurance circumstances.

Not every illness requires seeing a physician. However, the determination needs to be based on what the illness is, not based on whether the patient comes in the door with Medicare or Medicaid.

It just isn't the way to optimize use of health care dollars. There is also considerable fraud occurring around these issues.
