In very limited areas a practice can operate with only Medicaid and/or Medicare fees. For example, Mental Health clinics are vastly overpaid by Medicaids around the country, which has created rampant fraud in these areas. Similarly, NET (non-emergency transportation) has been been abused in many states.

However, as a general proposition, a physician's practice CANNOT SURVIVE on Medicare and Medicaid fees. Today, they aren't even paying fixed overhead costs for operating a medical practice. Which means that even shuffling patients to PAs and APNs is not a viable long-term arrangement.

Now you are making a very good case for single payer...which is the answer anyway...lots of "cogent arguments" around the world if you care to look.
