Perhaps "never happened" is a bit too strong, I mean something like "never happened in a clear way, in a large scale, in modern times". But that's probably close enough to "never happened", for the purposes of this discussion, some very old, and/or small, and/or obscure example is unlikely to impact people's opinions all that much.
