Cherry, I wished the problem was that simple. When you are enrolled in an HMO, you can only go to the pharmacies that they contract with, and can only get the meds on their formulary. But the problem is made much worse because you need a doctor who knows what he is doing to order the drugs. MCO's get around the requirement of providing care by not offering carved out contracts who Primary Care Hiv Physicians, eg: if I wanted to see HIV patients for Humana, I would have to get a Primary Care Contract, and see any patient who was sent since they "could not" guarantee me that only HIV patients came because they have not made a carve out. If, on the other hand I got a Specialist contract, that would mean I would only see patients when they were referred for only as many visits as they approve (usually 1 or 2), and only to really render an oppinion.

Thanks,

JLL
