Every product has a statistically certain failure rate. It was only a bad business decision if the potential statistically certain damages outweighed the costs of taking the product off the market and retooling, including soft-costs like damage to reputation of the company. I don't have that cost-benefit information. The exploding Ford Pinto comes to mind.
