Why are all black people called afro Americans when many of them came from the Caribbean and not from africa?

What about the white people who came to the United States from South Africa? Are they also called afro Americans or are they now just Americans?

Do you always, or at any time, refer to yourself as an Italian American on applications or to people in general when you describe yourself, or do you simply refer to yourself as an American? If not, then why not? TIA
