No making people lie involved. More like people who want you to pay something you don't want to pay for perpetrating a fraud in order to get it. Force (a government rule) or fraud, either way its an injustice against the employer.

Also I don't think it would be common. Most people aren't going to commit insurance fraud for such a modest expense.
