Something doesn't need to be universal to become the standard, only reach critical mass. Any benefit or entitlement decreed by the federal government automatically becomes the minimum standard. And we all expect to receive the minimum standard. That's just human nature.

You wouldn't reasonably have such an expectation when you work for an organization. that is strongly and publically against the "Ferrari"/coverage.

The employer's POV on Ferraris didn't matter back before the Ferrari law was passed. Once it's passed, the announcement is made, and you expect to get that Ferrari. Maybe only briefly, because then you find that your employer is fighting to deny it to you. You are not surprised that your employer is fighting for an exemption but your expectation was already established and you're still hoping that your employer won't prevail.

The effective raise would be small enough ($10 to $60 a month maybe)

According to the Kaiser estimate I posted, the additional cost per employee per year is under $20.

I imagine from the church's perspective the optics of the "we're not going to pay for contraception, but here's extra money that you could use to pay for contraception if you want to bonus" aren't good.

I think that "we're not going to pay for contraception but we want to make sure you're not being shortchanged on total compensation so here's an extra to spend as you please" would be just fine. It makes them sound thoughtful and fair while not compromising their principles. (That's an option I would recommend for consideration were I their management consultant.)
